Welcome to the Holy Cross Physician Partners Clinical Integration Network

Many aspects of the Patient Protection and Affordable Care Act (signed into law by President Obama in March 2010) will change in the next few months to years whether driven by political or legal forces.

One thing that will not change is the demand for a transition from a volumes based reimbursement system to a performance-based payment system where healthcare providers are held accountable for outcomes and efficiency. Physicians will be asked to comply with evidence-based practices as well as assume responsibility for positive, measured outcomes of these practices.

At the core of this true revolution in health care is the need to deliver the best outcomes in a cost-effective, efficient, reproducible and quantifiable manner: this is the heart of value-based medicine. The reason it will happen: the nation and the market demand it- there is simply not enough money to bear the cost increases of years past.

A group of physician leaders, private practice physicians, physicians employed by Holy Cross and several Holy Cross Hospital administrators have been meeting to discuss ways in which our medical community can work together to both respond to healthcare reform and to improve the quality and efficiency of care in our community.  This has been a powerful and informative process, during which we concluded that we need a physician-led healthcare model designed to benefit patients in our community, improve the coordination of care, reduce costs and sustain the finances of our local health care industry.  To accomplish this, we have formed a clinically integrated network of physicians, Holy Cross Hospital and affiliated health care providers Holy Cross Physician Partners (HCPP).
